Welcome to the Victory Physicians online Forms page. On this webpage you will find forms to speed up your visit and coordinate your care. Many of these forms are live and you may type your answers directly onto the form, save and print it. To save and print you will need to download the PDF applications at the bottom of this page and follow the instructions closely. It is very important that you fill out all appropriate forms before you arrive or time spent filling out forms will be deducted from your appointment slot time and you will have less face to face time with the doctor.

Print a PDF (Adobe Reader) file

To avoid printing blank pages, click the print icon within the Adobe Reader window instead of the browser’s print icon or File menu print options. If you have difficulty printing a particular PDF file, try saving it to your desktop or a flash drive first, and then print.





Save a PDF (Adobe Reader) file

Depending on your browser configuration, PDF files may appear in the browser window itself, or download automatically. The best way to save a PDF file that opens in your browser window is to click the save icon within Adobe Reader instead of using the browser’s “Save as” menu. The save icon looks like a small square floppy disk. After you save the blank form, double click on the new PDF document, on your hard drive, and complete the form. To save your completed form click on the save icon within Adobe Reader. (Most browsers will only save a blank form)
