Attention all patients. WE are now charging 25 cents per page to print out forms in our office. These do not include the standard registration forms. These do include pain forms and logs, POLST’s, durable power of attorneys, and advanced directives among others. The cost of paper and toner catridges is so high we cannot afford extra luxury printing such as these other forms and hence are now the responsibility of the patient.
